Le Rhumatisme Psoriasique (RhPso), rhumatisme inflammatoire chronique appartenant à la famille des spondyloarthrites, n'est pas seulement associé aux maladies inflammatoires (psoriasis, spondylarthrite ankylosante, uvéite et maladies inflammatoires chroniques de l'intestin), mais semble également accroître la prévalence de nombreuses comorbidités et notamment les maladies cardiovasculaires, l'obésité, le diabète, et a fortiori le syndrome métabolique, les maladies rénales, les hépatopathies non alcooliques, l'ostéoporose ou encore la dépression.
La prise en charge du RhPso a nettement évolué ces dernières années aux vues des nouvelles stratégies de soins et des nombreuses avancées thérapeutiques. Toutefois, les données concernant la persistance thérapeutique en vie réelle des molécules de seconde ligne, et notamment des "biological Drug Modifing the Activity of the Rheumatic Disease" (bDMARD), et les facteurs associés au maintien parmi les patients présentant un RhPso sont éparses.
Ces traitements peuvent également influer sur la consommation des molécules souvent co-prescrites (antalgiques, anti-inflammatoires non stéroïdiens [AINS], corticoïdes) et sur l'incidence de différentes comorbidités. Cependant les données concernant spécifiquement le RhPso sont très limitées.
Les objectifs de ce travail de thèse seront :
- D'estimer la persistance des biomédicaments en première ligne respectivement dans une cohorte de patients ayant un rhumatisme psoriasique et une cohorte de patients ayant un psoriasis puis comparer la persistance des différentes classes thérapeutiques dans chacune des cohortes.
- De rapporter et comparer la persistance à long terme des biomédicaments en deuxième ligne chez les patients atteints de rhumatisme psoriasique, ayant déjà été traités par anti-TNF.
- D'évaluer l'influence du sexe sur la persistance à long terme de chaque classe de thérapies ciblées.
